Licensed Vocational Nurse (LVN) Job Details:

Schedule: Monday - Friday | 8:00pm - 5:00pm

Location: Texas City, TX

Temporary to hire

Starting Pay Rate: $28.00/ hour + (based on experience)

Note: Due to providing care within male patient units and facility needs, male candidates are needed.

Job Responsibilities:

* Report directly to the Program Manager, Youth and Family Services, with oversight from the Chief Nursing Officer (CNO).
* Coordinate and manage medication clinics, including medication administration and documentation.
* Provide support to prescribing providers during patient visits and follow-up.
* Deliver patient and family education, including medication-related rehabilitative services that promote recovery.
* Monitor, document, and report patient progress, behavior, and response to treatment.
* Assist in the development and implementation of individualized care plans.
* Support crisis intervention efforts and ensure a safe therapeutic environment.
* Maintain accurate and timely medical records and nursing documentation in compliance with state and facility guidelines.
* Collaborate with multidisciplinary teams including physicians, therapists, social workers, and case managers to ensure coordinated care.
* Promote a respectful, compassionate, and supportive atmosphere for youth and families.

Education Requirements:

* High school diploma or equivalent, required.
* Active Texas LVN license required.

Job Requirements:

* Minimum of 6 months of nursing experience required..
* Strong interpersonal, communication, and organizational skills.
* Ability to work effectively within a team and adapt to changing patient needs.
* Commitment to patient-centered care and maintaining professional standards.
